If you are a new patient or have not been seen in the clinic since 2019, we ask that you complete the patient form on our website:

 www.waterdownoptometric.ca/new-patient.

Please be sure to fill this out 2-3 days prior to you scheduled appointment. Otherwise your appointment may have to be rescheduled.

We have a strict 24 hour cancellation policy. If you do not attend your appointment or cancel with fewer than 24 hours’ notice, you will be automatically charged a fee of $60 for the missed appointment.
